I just reviewed a patient case: on hormone pellets for 3 years, tested yearly, and still not truly assessed for hormone activity. Here’s what went wrong—and what to ask for instead.

• Annual “bloodwork” can be routine panels—not hormone-specific testing
• Even when estrogen/testosterone are drawn, blood levels may not reflect tissue-level activity
• Hormones act in fat, brain, and muscle—not just as numbers in circulation
• If testing doesn’t measure what’s active, providers often end up relying on symptoms
• Saliva testing can be a more direct way to see what your body is actually doing

If you’re on pellets or hormone therapy, check what you’re being tested for. What lab method does your provider use?
